The Ohio Department of Health implemented the Creating Healthy Communities Program-funded predominantly by the Preventive Health and Health Services Block Grant-to improve the health of Ohioans and prevent chronic disease statewide by increasing opportunities for physical activity, improving access to and affordability of healthy food, and ensuring tobacco-free living. In 2016, the CHC program impacted more than 700,000 Ohioans in 23 counties (both rural and urban) and leveraged more than $4 million to support these sustainable changes.
